Domestic Violence Forms

The forms required for Domestic Violence Civil Protection Orders are provided to our Court by the Supreme Court of Ohio. It is required by law that certain documents be filed when petitioning the Court for a Domestic Violence Civil Protection Order. All forms must be completed fully and accurately. Once a document is filed with the Clerk of Court's Office the document cannot be changed.

If you are filing without an attorney, you will only need to prepare the necessary forms and a statement describing the acts of domestic violence for the Court. You may click on the Forms Help button above to complete the forms before you arrive at court, or you will be helped with these forms by court staff when you arrive. The forms you complete using the Forms Help option may be printed at court when you arrive. Go to the DV Help Center located in room 53 on the ground floor of the Cuyahoga County Courthouse, 1 W.
